Transitional Navigation
Our Process:
At Highest Potentials, our priority is building a positive relationship with your family. Families will complete an introductory form containing essential information about their loved one's disability, medical diagnosis, educational history, progress reports, any existing 504 plans or IEPs and short and long-term goals for our collaboration.

In your family's pre-consultation we work together collaboratively to identify goals and gaps in your adult child's post-secondary transitional plan and what supports are available locally to help bridge those gaps. This becomes your roadmap for navigating the disability support system in your city and state.

The level of support your family desires determines our next steps. Our three levels of ongoing support are listed on this page. All levels include the pre-consultation stage and creation of your family's customized roadmap with clear steps to follow to access services and supports.

Additional consultations and add-on services are always available at any tier for a one-time fee.

I do
Our highest level of support. After filling out an introductory form and clarifying your adult child's transitional goals during a pre-consultation, we provide a customized roadmap and handle all the execution, updating you along the way.
This tier includes done-for-you support filling out forms, applying for services and preparation support and attendance at any meetings needed. Also includes email and text support during business hours.

We do
A collaborative approach: After filling out an introductory form and clarifying your adult child's transitional goals during a pre-consultation, we provide a customized roadmap and as needed support as you work through filling out paperwork and applying for services.
Includes email and text support during business hours and optional add-on consultations when requested at a reduced hourly rate.

This approach is for those who wish to DIY much of the process. Following our pre-consultation, you will recieve a detailed, custom roadmap with the steps required to acheive your adult child's transitional goals. You'll know exactly what needs to be done and in what order. Includes support answering clarifying questions about your plan. Additional consultations and support available as hourly add-ons.

From one of our parents...
My son is 21 years old. I came to Barbara at Highest Potentials needing support knowing which way was up in terms of applying for services for our adult son.
There’s so many things to know and so many papers to fill out – some of them very redundant – so just knowing I had the support of an expert who knew exactly what we needed to do and in what order was a huge relief.
I’ve spent my son’s whole life being his cheerleader and advocate so having to get in the headspace of really listing out super specifically what he struggles with and his challenges and weaknesses in just the right way to get him the support he needs to live on his own was a big challenge for me. Having Barbara’s expert eye on everything throughout the process left no stone unturned in terms of accessing the help he needed.
Since he’s higher functioning and was always in a general ed classroom, he is truly capable of doing a lot on his own so Barbara really helped me realize that just because he doesn’t struggle as much as others doesn’t mean I shouldn’t get him the services he really needs to thrive.
It was so helpful to know what types of supports were available, how to access them, and just the reminder that he DOES deserve that level of support. And I deserve to have him receive that level of support – I don’t always have to be the one giving and giving out of my own cup.
We’ve all had to do it. We’ve all had to be the mama bear. I think sometimes as moms we are doing so much and we don’t always realize it’s okay to ask for support.
